AIDS CURE

A few patients (menos del 1%) who spontaneously control their viremia to undetectable levels, a female baby, a German who received a bone marrow transplant and 10 men and 4 women, with temporary (functional) cures, make a handful of patients HIV-1 + (sustained viral replication, progressive loss of CD4 + T, ending in AIDS), have achieved a temporary functional cure (reducing size of reservoirs HIV, low viral levels, decrease in the number of CD4 T, with clonogenic properties or long life: naïve and central memory T cells), of this dreaded disease. The observations and treatments performed in USA, Germany and France are important, because they force us to rethink the focus of research and question the generalization of antiretroviral therapy (cART), characterized by suppress viral replication and reduce drastically the morbidity and mortality. The cART not eradicate infected cells and discontinuing it, plasma viremia quickly reappears. The cART temporarily restores immunity at the expense of ingesting medication and suffer lifelong side effects.

In the 14 French receiving early the cART early (primary infection by HIV-1), was observed limitation of viral reservoirs and extent of viral mutations, with preservation of immune responses, to the point of suspending the therapy for up to 7 years, although not disappearing the AIDS. With cART, the cure is difficult because the virus hides in reservoirs and nests (sleeper), cells unidentified as infected by the immune system, remaining the patient in a state of HIV-1 +, for years. Giving early cART, is possible after some years, full suspension therapy. In our opinion, the referred cases suggest that the mechanism of permanent or functional cure of HIV-1 patients is exerted by immune mechanisms. Although most cART destroys viruses, a few are hidden. The destruction of these or ultimate control is in charge of immune mechanisms to discover. Little is known of the normal and pathological immune functioning. We envision the future replacement of cART by a vaccine as definitive therapy for this disease. We need to know more about the immunology of the newborn, in adults, elderly, more comparative and evolutionary immunology.

CURA DEL SIDA

Un escaso numero de pacientes (less than 1 %), que controlan espontáneamente sus viremia hasta niveles indetectables, una bebe de sexo femenino, un alemán que recibió un trasplante de medula osea y 10 hombres y 4 mujeres, con curas temporales (funcionales), conforman un puñado de pacientes HIV-1 + (replicacion viral sostenida, perdida progresiva de células CD4+ T, terminando en SIDA), que han logrado una cura funcional temporal (reducción del tamaño de reservorios HIV, niveles virales bajos, disminución del numero de células CD4 T, con propiedades clonogénicas o de larga vida: naïve y células centrales T memoria), de la temida enfermedad. Las observaciones y tratamientos realizados en USA, Alemania y Francia interesan porque obligan a replantear el foco de las investigaciones y cuestionar la generalización de la terapia antirretroviral (cART), caracterizada por suprimir la replicacion viral y disminuir drasticamente la morbilidad y mortalidad. El cART no erradica las celulas infectadas y al suspender el tratamiento reaparece rápidamente la viremia plasmatica. El cART restaura temporalmente la inmunidad, a costa de ingerir la medicación toda la vida y sufrir efectos colaterales (algunos letales).

En los 14 franceses que recibieron tempranamente el cART (infeccion primaria por HIV-1), se observo limitación de los reservorios virales y de la extensión de las mutaciones virales, con preservación de las respuestas inmunes, al punto de suspenderse la terapia por hasta 7 años, aunque el SIDA no desapareciese. Con el cART la cura es dificil porque los virus se esconden en reservorios y nidos de celulas durmientes no identificadas como infectadas por el sistema immune, permaneciendo el paciente HIV-1, en estado de positividad, por años. Administrando cART tempranamente, es posible tras algunos años, suspender totalmente la terapia. Según nuestra opinión, los casos referidos sugieren que el mecanismo de cura funcional o definitiva del SIDA, es ejercido por mecanismos inmunes. Aunque el cART destruye la mayoría de los virus, unos pocos quedan ocultos. La destrucción de estos o su control definitivo queda a cargo de mecanismos inmunes por descubrir. Se conoce poco del funcionamiento inmune normal y patológico. Avizoramos la sustitución del cART por una vacuna como terapia definitiva de este mal. Necesitamos conocer mas de la inmunología del recién nacido, del adulto, del adulto mayor, mas de inmunologia comparada y evolutiva.
