Controlling generalized seizures (controlando epilepsias).

Bautista Lastres, historian par excellence of the Peruvian medicine, interpreted the drawing of the left (Taken from the Nueva Corónica and http://www.kb.dk/elib/mss/poma/, Buen Gobierno-1615-1616, of Felipe Guamán Poma de Ayala), as an epilepsy attack (great seizures), happening to the Fifth Colla (wife of the fifth Peruvian Inca) (1). Later, about times near Conquest, certain pre-Hispanic religious groups thought that epiléptic people talked with the God Sun, during the attacks. The Greeks thought that the disease seated in the heart and, until well advanced century XVII, many European thought that a person with epiléptic attacks, was full of malignant spirits. Until shortly before the sexual revolution of this century, some people attributed the epiléptic attacks, to an excess of masturbation practices. The cases of famous epiléptics are numerous: Gustave Flaubert, Dostowiesky, King Saúl, Hercules, Juana de Arco, Alexander the Magnificient, Luis II, Carlos V, Richelieu, Lord Byron, Alfred Nóbel, Lenin, Pio X, Helmholtz, among others. The remarkable thing is that no of the named ones, accused intellectual reduction.

One calculates that they exist: 200 000 epiléptics in all Peru; 48 000, of them, in Lima. http://www.minsa.gob.pe/ocom/notaanterior.asp?np_codigo=2125. Although the number seems exaggerated, it is not. What happens is that generally we thought that epiléptic are only those people who suddenly fall to the ground, lose the conscience and had convulsive attacks (generalized epilepsies, ritmic corporal movements, generalized, crude). The these cases, (the minuses), and most severe, correspond both to ample pathological affectation of nervous cells in both cerebral hemispheres. Most of cases, nevertheless, it corresponds more to located commitments, usually in a single cerebral area (partial epilepsies), in which the patients become disconnected suddenly of their surroundings, per seconds, do not fall to the floor, nor lose the conscience. In these cases -often unnoticed-, the patients exhibit strange conducts, show recurrent confusion, lose the concentration momentarily (most in children), accuse sensation of incrustation of needles or, certain creeps of corporal parts, disagreeable flavor front to certain meals, partial convulsions of an arm or, part of him. The causes of the epilepsy in general, are varied, but it admits that the people who have previous cerebral insults, have more probability of developing epiléptics attacks.

People exist who suffer so up to 15 daily epiléptics attacks in such way that the affection prevents them to be related socially, to study, to mobilize itself, or to marry. Some of them die during the dream. The medicines improve most of the slight cases, but they show difficulties to domesticate the most severe cases called refractory. In these, one resorts to the cerebral surgical removal of the affected part, not always successfully. These operations diminish the attacks, but often leave sequels disagreeable (difficulty to see, to speak, etc), reason why they are not entirely satisfactory.

The good news is that in the recent last years it tends to work (as always had to be), with precise knowledge but of the cerebral function. Now it is known that the cerebral activities of a normal person, massively stimulate certain group of nervous cells, those that when being excited unload serial energy, to the neighboring cells. Simultaneously, another great cellular group, struggles to inhibit them. From this fight the normal, trasmisible power amount emerges. When the unloading happens, without the counterbalance neuronal corresponding, the epiléptic attacks are generated. Now very small magnets hung on the neck of the epiléptic people have begun to be used, http://news.bbc.co.uk/1/hi/england/lincolnshire/4318820.stm, connected in permanent form to cerebral nerves (vagus nerve). The magnetic impulses, when acting like permanent cerebral inhibitors, abort the attacks. The first results of this technique, are quite encouraging.

Los griegos pensaban que el mal asentaba en el corazón y, hasta bién entrado el siglo XVII, muchos europeos pensaban que una persona con ataques epilépticos, estaba poseída por espíritus malignos. Hasta poco antes de la revolución sexual de principios de siglo, algunas personas achacaban los ataques epilépticos, a un exceso de prácticas maturbatorias. Son numerosos los casos de epilépticos famosos: Gustave Flaubert, Dostowiesky, El Rey Saúl, Hércules, Juana de Arco, Alejandro el Magno, Luis II, Carlos V, Richelieu, Moliere, Lord Byron, Alfred Nóbel, Lenin, Pio X, Helmholtz, entre otros. Lo notable es que ninguno de los nombrados, acusó menoscabo intelectual.
Se calcula que existen : 200 000 epilépticos en todo el Perú ; 48 000, de ellos, en Lima. Aunque la cifra parezca exagerada, no lo és. Lo que sucede es que generalmente contabilizamos como epilépticas tan solo a aquellas personas que súbitamente caen al suelo, pierden la conciencia y convulsionan (epilepsias generalizadas, con movimientos corporales ritmicos, generalizados, groseros). Estos casos, (los menos), y los más severos, corresponden a afectacion patológica amplia de células nerviosas en los dos hemisferios cerebrales. La mayoría de casos, sin embargo, corresponde a compromisos más localizados, usualmente en una sola área cerebral (epilepsias parciales), en las que los pacientes se desconectan súbitamente de su entorno, por segundos, no caen al piso, ni pierden la conciencia. En estos casos -muchas veces desapercibidos-, los pacientes exhiben conductas extrañas, muestran confusión recurrente, pierden momentáneamente la concentración (niños), acusan sensación de incrustación de agujas o, hormigueo de cierta partes corporales, sabor desagradable frente a ciertas comidas, convulsiones parciales de un brazo o, parte de él. Las causas de la epilepsia en general, son variadas, pero se admite que las personas que tienen antecedentes de injurias cerebrales previas, tienen más probabilidad de desarrollar ataques epilépticos.
Existen personas que padecen hasta 15 ataques epilépticos diarios de modo tal que la afección les impide relacionarse socialmente, estudiar, movilizarse, o casarse. Algunos de ellos mueren durante el sueño. Los medicamentos mejoran la mayoría de los casos leves, pero muestran dificultades para domesticar los casos más severos o, refractarios. En estos, se recurre a la remoción quirúrgica cerebral de la parte afectada, no siempre con éxito. Estas operaciones disminuyen los ataques, pero muchas veces dejan secuelas desagradables (dificultad para ver, hablar, etc), por lo que no es enteramente satisfactoria.
La buena noticia es que en los ultimos años se tiende a trabajar (como siempre debió ser), con conocimientos mas precisos del funcionamiento cerebral. Ahora se conoce que las actividades cerebrales de una persona normal, estimulan masivamente a cierto grupo de células nerviosas, las que al excitarse descargan energía seriada, a las células vecinas. Simultáneamente, otro gran grupo celular, pugna por inhibirlas. De esta lucha emerge la cantidad energética normal, trasmisible. Cuando la descarga ocurre, sin el contrapeso neuronal correspondiente, se generan los ataques epilépticos. Ahora han empezado a utilizarse minúsculos magnetos colgados del cuello de las personas epilépticas, conectados a su véz, en forma permanente a nervios cerebrales (n. vago). Los impulsos magnéticos, al actuar como inhibidores cerebrales permanentes, abortan los ataques. Los primeros resultados de esta técnica, son bastante alentadores.
