World fights against Malaria: to the long one the things will be worse.

The scientific news of the last week is the appointment of the new Boss of the World Health Organization (WHO), against the Malaria : Arata Kochi, a Japanese, direct stubborn, of few words, well-known as the iron man in the high spheres of the world health. Kochi, was until recently Boss of the fight against the Tuberculosis (Stop TB Initiative), of the WHO. Their methods of that then don't seem to differ of those that he now will use in their world fight against the Malaria: effective at short term, Jurassic and terrible at long term (what really is of worth). As we notice it in a previous article, Kochi, strongly imposed the standardized treatment (sputum exam, absence of exam of sensibility to medications, treatment with 4 antituberculostatic medications: DOTS, during 6 months), in most developing countries, with the already well-known consequences : upward emergency of almost incurable, omniresistent strains.

Now, in charge of the World Program of fight against the Malaria, Kochi, has just prohibited the employment of the chinese antimalaric : artemisinin (... " monotherapy, will create resistant strains"..." we cannot use herbs against the malaria"..). Kochi also plans to sprays the inner walls of African houses affected with malaria with DDT, a cheap insecticide, without patent’s problems, although feared to contaminate african export organic products and being a potential carcinogenic. For us the presence of Kochi to the front of the antimalaric fight is controversial, more political than scientific based. We already know what will happen : to short term, the numbers of cases of malaria will descend visibly ; at the long term the consequences will be catastrophic : emergency of Plasmodiums and mosquitoes ultraresistance strains. Consequence of care, because antimalaric medications take 10 years in being developed. For us, so much in the Malaria, TBC, AIDS and others, the solution but important and definitive is the generation of new types of vaccines (from DNA, etc). To this aim -in Peru- like in the high spheres of the world health, seem to oppose bigger economic interests of certain transnational drug developers, certain lobbies and political guidelines. In situations like this, do we always asked ourselves : what percentage of the money that contributes our country and certain international institutions to solve our problems of health, truly arrives to the sick peruvian users?. For the inefficacy of the results, we estimate that less than 20%.

Lucha mundial contra la Malaria :a la larga las cosas seran peores.

La noticia cientifica de la semana que pasó, la constituye el nombramiento del nuevo Jefe de la Organización Mundial de la Salud (OMS), contra la Malaria, Arata Kochi, un tozudo japonés, directo, de pocas palabras, conocido como el hombre de hierro en las altas esferas de la salud mundial. Kochi, fué hasta hace poco Jefe de la lucha contra la Tuberculosis (Stop TB Initiative), de la OMS. Sus métodos de ese entonces no parecen diferir de los que ahora empleará en su lucha mundial contra la malaria : eficaces y efectistas a corto plazo, jurásicas y pésimas a largo plazo : que es lo que realmente importa. Como lo hicimos notar en un articulo anterior, Kochi, impuso férreamente el tratamiento estandarizado (examen de esputo, ausencia de examen de sensibilidad del germen, tratamiento con 4 medicamentos antituberculosos : DOTS, durante 6 meses), en la mayoria de paises en vias de desarrollo, con consecuencias ya conocidas : emergencia in crescendo de cepas omniresistentes, casi incurables.

Encargado del Programa Mundial de lucha contra la Malaria, Kochi, acaba de prohibir el empleo del antimalarico de origen chino : artemisinina (…”la monoterapia, creara cepas resistentes”….”no podemos usar yerbas contra la malaria”….). Ahora, Kochi tambien planea rociar las paredes interiores de las casas africanas afectas por la malaria con DDT, un insecticida barato, sin problemas de patente, aunque temido por contaminar productos orgánicos africanos de exportación y ser un cancerigeno potencial. Para nosotros la presencia de Kochi, al frente de la lucha antimalárica, es controversial, obedeciendo a intereses politicos mas que a motivaciones cientificas. Ya sabemos lo que sucederá a corto plazo : el numero de casos de malaria descenderá visiblemente ; a largo plazo las consecuencias seran catastróficas, emergencia de cepas de Plasmodiums y mosquitos ultraresistentes. Consideraciones de cuidado, porque los medicamentos antimalaricos tardan 10 años en desarrollarse. Para nosotros, tanto en la Malaria, TBC, AIDS y otros, la solucción más importante y definitiva es la generacion de nuevos tipos de vacunas (a partir del DNA, etc). A ello -en el Perú- al igual que en las altas esferas de la salud mundial parecen oponerse intereses económicos de ciertas transnacionales farmaceuticas, lobbies grupales y lineamientos politicos. En situaciones como esta, siempre nos preguntamos que porcentaje del dinero que aporta nuestro pais y ciertas instituciones internacionales para soluccionar problemas de salud, llega verdaderamente a los usuarios peruanos finales?. Por la ineficacia de los resultados, estimamos que menos del 20 %.
