TRYING to DEFEAT TUBERCULOSIS


Every year the tuberculosis (TBC), kills world wide: 2 million people and infects other 8. Although most cases are cured, a growing resistance exists to first and second line drugs. In nations in development 75% of cases happens between the 15 and 54 years. The answer to such threat are biomolecular strategies gestated during interactions among bacterias and human beings, the same ones that are generating new therapies and diagnostic tests. Mycobacterium tuberculosis (Mt), exists in 2 forms: latent and active. We speak of latent form when the immune system prevents that the bacteria multiplies, persisting per decades, without the guest makes sick. Alone 10% of infected people (with HIV, defenses diminished, chemotherapy), develops the active form of the illness. Without treatment, half of those infected dies with lung destruction. The current treatment against active forms includes: isoniazide, ethambutol, pyrazinamide and rifampicine. 5% of the 8 million new cases, are resistant stumps from Mt to isoniazide and rifampicine. Most of multi-drug-resistant TBC (MDR-TB), are cured with drugs of second line with treatments that last 2 years and produce severe side effects. Today, there are increasing cases of extensive drug resistance TBC (XDR-TB), to any drug of second line.

I) Among the new drugs the SQ109, inhibit the synthesis of cellular wall and the PA-824, attack the Mt in their stages of slow and active division, shortening the time of treatment II) Until 15 years ago the first step was to identify essential enzymes for the survival of the bacteria, then their inhibitors were generated. III) When being known the genome of Mt in 1998, it was discovered that Mt synthesized and degraded lípids and that Mt was able to live in absence of air, slowing its metabolism, process that makes Mt less sensitive to antibiotics IV) It has been discovered that attenuated Mt (of vaccines), lack a segment of DNA, containing essential genes for the virulence. V) Now all the processes and cellular components are studied (proteómic), the quantity of RNA messenger of each gene (transcriptómic) and the intermediate and final products of cellular metabolism (metabolómic). When Mt is treated with PA-824, its transcriptomic reacts as if it had been poisoned with KCN. VI) The Mt produces NO2, a defensive molecule generated by human immune cells. The structural genomics search to determine the 3D structure of each protein of Mt, which will help to design and to synthesize new drugs.

VII) Today, instead of studying a well-known protein and its functional inhibitor, they are studied compound that inhibit the reproduction of Mt in cellular cultures identifying also the deteriorated enzyme. VII) before the Mt invaded human bodies, they occupied environmental niches, where they competed with countless bacterias that maintained them to line. It cares today, to identify these bacterias. VIII) The bioinformátic is discovering emergent properties of Mt (cords, as a consequence of complex interactions among bacterial superficial molecules). Emergent property are the granulomas formation, which difficult drug penetration. IX) With the bioinformátic, it is hoped to determine the relationships among the 4,000 genes of Mt, their corresponding proteins and their bacterial metabolic products. X) it is tried to reply in computer a silice Mt, to choose the best drugs and possible emergent metabolic routes. XI) it is continued looking for a vaccine, constituted by a Mt, not attenuated. With the genomas of Homo sapiens and of M. tuberculosis, to our disposal the moment of the great change has arrived.
COMBATIENDO la TUBERCULOSIS
Cada año la tuberculosis (TBC), mata 2 millones de personas en el mundo e infecta otras 8. Aunque la mayoria de casos son tratables, existe una creciente resistencia a drogas de primera y segunda línea. En naciones en desarrollo el 75 % de casos ocurre entre los 15 y 54 años. Ante tal amenaza, estrategias biomoleculares gestadas durante el estudio de interacciones entre bacterias y humanos están generando nuevas terapias y tests diagnósticos. Mycobacterium tuberculosis (Mt), se encuentra en 2 formas : latente y activa. Hablamos de formas latentes cuando el sistema inmune impide que la bacteria se multiplique y persista por décadas, sin que el huésped enferme. Solo un 10 % de gente infectada (con HIV, disminución de defensas, quimioterapia), desarrolla la forma activa de la enfermedad. Sin tratar, la mitad de los infectados muere, mayormente con destrucción pulmonar. El tratamiento actual involucra 4 drogas de primera linea, muy activas contra formas activas : isoniazida, etambutol, pirazinamida y rifampicina. El 5% de los 8 millones de casos nuevos, son cepas resistentes de Mt contra isoniazida and rifampicina. La mayoría de TBC multi-drogo-resistente (MDR-TB) son tratables, con drogas de segunda linea con tratamientos que duran 2 años y producen severos efectos colaterales. Hoy, estan aumentando los casos de TBC de extensa resistencia a drogas (XDR-TB), a virtualmente cualquier droga de segunda linea .

I) Entre nuevas drogas, la SQ109, inhibibe la síntesis de pared celular y la PA-824, ataca al Mt en sus estadios de división lenta y activa, acortando el tiempo para tratar la enfermedad II) Hasta hace 15 años primero se identificaban las enzimas esenciales para la supervivencia de la bacteria y luego inhibidores a estas enzimas. III) Al conocerse el genoma de Mt en 1998, se conocieron todas las enzimas y las reacciones quimicas requeridas para la supervivencia de Mt en humanos. Ahora se sabe que Mt sintetiza y degrada lipidos y que vive en ausencia de aire enlenteciendo su metabolismo lo que lo hace menos sensible a antibioticos IV) Se ha descubierto que a los Mt de las vacunas, les falta un segmento de DNA, con genes esenciales para la virulencia. V) Ahora se estudian todos los procesos y componentes celulares (proteomica), la cantidad de RNA mensajero de cada gene (transcriptomica), hasta productos intermedios y finales del metabolismo cellular (catabolomica). Cuando Mt es tratado con PA-824, el transcriptoma reacciona como si hubiera sido envevenado con KCN. VI) Mt libera NO2, una molecula defensive generada por celulas inmunes humanas. La genomica structural busca determinar la estructura 3D, de cada proteina de Mt, lo que ayudara a diseñar y sintetizar nuevas drogas. VII) Ahora, en lugar de estudiar una proteina conocida y su inhibidor funcional, se estudian compuestos que inhiben la reproducción de Mt en cultivos celulares, identificando la enzima deteriorada por la sustancia VII) Antes que las Mt invadieran cuerpos humanos, ocupaban nichos medioambientales, donde competian con innumerables bacterias que las mantenian a raya. Seria importante identificar a estas bacterias. VIII) El desarrollo de la bioinformática esta descubriendo propiedades emergentes de Mt (cordones, a consecuencia de interacciones complejas entre moleculas de la superficie bacteriana y desarrollo no predecible). Una propiedad emergente es la formación de granulomas donde difícilmente penetran las drogas. IX) Con la bioinformatica, se espera determinar las relaciones existentes entre los 4,000 genes de Mt, sus correspondienjtes proteinas y sus productos metabólicos bacterianos. X) Finalmente, se intenta replicar en computadora un Mt de silicona para escoger las mejores drogas y las posibles rutas metabólicas emergentes. XI) Se sigue buscando una vacuna, aunque no con versiones atenuadas de Mt. Estando a nuestra disposicion los genomas del Homo sapiens y de M. tuberculosis, el momento del gran cambio ha llegado.
