TRYING to DEFEAT TUBERCULOSIS


Every year the tuberculosis (TBC), kills world wide: 2 million people and infects other 8. Although most cases are cured, a growing resistance exists to first and second line drugs. In nations in development 75% of cases happens between the 15 and 54 years. The answer to such threat are biomolecular strategies gestated during interactions among bacterias and human beings, the same ones that are generating new therapies and diagnostic tests. Mycobacterium tuberculosis (Mt), exists in 2 forms: latent and active. We speak of latent form when the immune system prevents that the bacteria multiplies, persisting per decades, without the guest makes sick. Alone 10% of infected people (with HIV, defenses diminished, chemotherapy), develops the active form of the illness. Without treatment, half of those infected dies with lung destruction. The current treatment against active forms includes: isoniazide, ethambutol, pyrazinamide and rifampicine. 5% of the 8 million new cases, are resistant stumps from Mt to isoniazide and rifampicine. Most of multi-drug-resistant TBC (MDR-TB), are cured with drugs of second line with treatments that last 2 years and produce severe side effects. Today, there are increasing cases of extensive drug resistance TBC (XDR-TB), to any drug of second line.

I) Among the new drugs the SQ109, inhibit the synthesis of cellular wall and the PA-824, attack the Mt in their stages of slow and active division, shortening the time of treatment II) Until 15 years ago the first step was to identify essential enzymes for the survival of the bacteria, then their inhibitors were generated. III) When being known the genome of Mt in 1998, it was discovered that Mt synthesized and degraded lípids and that Mt was able to live in absence of air, slowing its metabolism, process that makes Mt less sensitive to antibiotics IV) It has been discovered that attenuated Mt (of vaccines), lack a segment of DNA, containing essential genes for the virulence. V) Now all the processes and cellular components are studied (proteómic), the quantity of RNA messenger of each gene (transcriptómic) and the intermediate and final products of cellular metabolism (metabolómic). When Mt is treated with PA-824, its transcriptomic reacts as if it had been poisoned with KCN. VI) The Mt produces NO2, a defensive molecule generated by human immune cells. The structural genomics search to determine the 3D structure of each protein of Mt, which will help to design and to synthesize new drugs.

VII) Today, instead of studying a well-known protein and its functional inhibitor, they are studied compound that inhibit the reproduction of Mt in cellular cultures identifying also the deteriorated enzyme. VII) before the Mt invaded human bodies, they occupied environmental niches, where they competed with countless bacterias that maintained them to line. It cares today, to identify these bacterias. VIII) The bioinformátic is discovering emergent properties of Mt (cords, as a consequence of complex interactions among bacterial superficial molecules). Emergent property are the granulomas formation, which difficult drug penetration. IX) With the bioinformátic, it is hoped to determine the relationships among the 4,000 genes of Mt, their corresponding proteins and their bacterial metabolic products. X) it is tried to reply in computer a silice Mt, to choose the best drugs and possible emergent metabolic routes. XI) it is continued looking for a vaccine, constituted by a Mt, not attenuated. With the genomas of Homo sapiens and of M. tuberculosis, to our disposal the moment of the great change has arrived.
COMBATIENDO la TUBERCULOSIS
Cada año la tuberculosis (TBC), mata 2 millones de personas en el mundo e infecta otras 8. Aunque la mayoria de casos son tratables, existe una creciente resistencia a drogas de primera y segunda línea. En naciones en desarrollo el 75 % de casos ocurre entre los 15 y 54 años. Ante tal amenaza, estrategias biomoleculares gestadas durante el estudio de interacciones entre bacterias y humanos están generando nuevas terapias y tests diagnósticos. Mycobacterium tuberculosis (Mt), se encuentra en 2 formas : latente y activa. Hablamos de formas latentes cuando el sistema inmune impide que la bacteria se multiplique y persista por décadas, sin que el huésped enferme. Solo un 10 % de gente infectada (con HIV, disminución de defensas, quimioterapia), desarrolla la forma activa de la enfermedad. Sin tratar, la mitad de los infectados muere, mayormente con destrucción pulmonar. El tratamiento actual involucra 4 drogas de primera linea, muy activas contra formas activas : isoniazida, etambutol, pirazinamida y rifampicina. El 5% de los 8 millones de casos nuevos, son cepas resistentes de Mt contra isoniazida and rifampicina. La mayoría de TBC multi-drogo-resistente (MDR-TB) son tratables, con drogas de segunda linea con tratamientos que duran 2 años y producen severos efectos colaterales. Hoy, estan aumentando los casos de TBC de extensa resistencia a drogas (XDR-TB), a virtualmente cualquier droga de segunda linea .

I) Entre nuevas drogas, la SQ109, inhibibe la síntesis de pared celular y la PA-824, ataca al Mt en sus estadios de división lenta y activa, acortando el tiempo para tratar la enfermedad II) Hasta hace 15 años primero se identificaban las enzimas esenciales para la supervivencia de la bacteria y luego inhibidores a estas enzimas. III) Al conocerse el genoma de Mt en 1998, se conocieron todas las enzimas y las reacciones quimicas requeridas para la supervivencia de Mt en humanos. Ahora se sabe que Mt sintetiza y degrada lipidos y que vive en ausencia de aire enlenteciendo su metabolismo lo que lo hace menos sensible a antibioticos IV) Se ha descubierto que a los Mt de las vacunas, les falta un segmento de DNA, con genes esenciales para la virulencia. V) Ahora se estudian todos los procesos y componentes celulares (proteomica), la cantidad de RNA mensajero de cada gene (transcriptomica), hasta productos intermedios y finales del metabolismo cellular (catabolomica). Cuando Mt es tratado con PA-824, el transcriptoma reacciona como si hubiera sido envevenado con KCN. VI) Mt libera NO2, una molecula defensive generada por celulas inmunes humanas. La genomica structural busca determinar la estructura 3D, de cada proteina de Mt, lo que ayudara a diseñar y sintetizar nuevas drogas. VII) Ahora, en lugar de estudiar una proteina conocida y su inhibidor funcional, se estudian compuestos que inhiben la reproducción de Mt en cultivos celulares, identificando la enzima deteriorada por la sustancia VII) Antes que las Mt invadieran cuerpos humanos, ocupaban nichos medioambientales, donde competian con innumerables bacterias que las mantenian a raya. Seria importante identificar a estas bacterias. VIII) El desarrollo de la bioinformática esta descubriendo propiedades emergentes de Mt (cordones, a consecuencia de interacciones complejas entre moleculas de la superficie bacteriana y desarrollo no predecible). Una propiedad emergente es la formación de granulomas donde difícilmente penetran las drogas. IX) Con la bioinformatica, se espera determinar las relaciones existentes entre los 4,000 genes de Mt, sus correspondienjtes proteinas y sus productos metabólicos bacterianos. X) Finalmente, se intenta replicar en computadora un Mt de silicona para escoger las mejores drogas y las posibles rutas metabólicas emergentes. XI) Se sigue buscando una vacuna, aunque no con versiones atenuadas de Mt. Estando a nuestra disposicion los genomas del Homo sapiens y de M. tuberculosis, el momento del gran cambio ha llegado.

BIZARRE ELECTRONIC SPIN


A group of international scientists led by physicists from Princeton University have filmed the strange behavior of electronic spin inside alloy of bismuth with antimony, which could lead to greater data storage capacity, improvements in electronics, more computing speed, better studies of quantum behavior, study of unusual behavior of magnets, superconductors and others, said Zahid Hasan, Princeton professor of physics (Science, Feb. 13).

In the experiment most electron returns to its normal position after giving 2 full turns (720 degrees), instead of the usual 360 degrees. In quantum domains a object can be in 2 places at the same time, also there is no distinction between waves and particles. Physicists agree that these bizarre circuits represent added alternatives to the usual and simple "on" and "off". The experiment has allowed the development of imaging techniques to follow quantum behavior. Now you can take photos of electronic spin, as they travel through the north and south poles of small intraelement magnets, hoping that this knowledge will improve medical images.

EXTRAÑA CONDUCTA DE SPIN ELECTRONICOS

Un grupo de científicos internacionales liderados por físicos de la Universidad de Princeton, han filmado la extraña conducta de spines electrónicos al interior de aleaciónes de bismuto con antimonio, que podrían conducir a una mayor capacidad de almacenamiento de datos, mejoras en la electrónica, mas velocidad de computación, estudios mejorados de conducta cuántica, conducta inusual de magnetos, superconductores y otros, dice Zahid Hasan profesor de fisica de Princeton (Science, Feb. 13).

En el experimento la mayoría de electrones retorna a su posición normal luego de dar 2 vueltas completas (720 grados), en véz de los 360 grados usuales. En dominios cuánticos un objeto puede estar en 2 lugares al mismo tiempo, no existiendo distinción entre ondas y partículas. Los físicos concuerdan en que estos circuitos bizarros, representan alternativas agregadas a los usuales y simples "on" y "off". El experimento ha permitido desarrollar además técnicas imagenológicas para objetivar la conducta cuántica. Ahora es posible tomar fotos del spin electrónico, mientras estos viajan por los polos norte y sur de pequeños magnetos intraelementos, esperándose que estos conocimientos mejoren la imagenología médica.

BLOOD TRANSFUSION and JEHOVA WITNESSES


Through RPP (Radioprograms of Peru. 7, 30 pm. 20/2/09), we listened about the sudden liver trauma (plows and bled of liver), of a 23 year-old woman interned in a hospital of Trujillo-Peru that with 2 g of hemoglobin (Hb), refused to be transfused with total blood, tending to stabilize her while being operated. Through RPP, close relatives spoken supporting the patient's position (follower of Jehova's Witnesses, that prohibited transfusions of human blood), doctors, lawyers and country attorneys obstinated in transfusing her total blood, because the patient was dying. All defended the position of the doctors, none trying to understand the patient's apparent absurdity. It is not the first time that this happens in Peru. Although we called several times to RPP, we desisted because this radio is not of free participation, it is not like the blogs.

I) It was important to know the patient's graveness (arterial pressure, breathing frequency, pulse, urine flow, etc). Not rarely we see patients with 2 g of Hb (renal inadequacy, multiparasitised, aplastic anemia, etc) walking, without risk of life. Although these are chronically adapted patients -in science- it cares to demonstrate why somebody has their life in risk. II) It also cares to meditate on the axis of the process or illness and its possible alternatives. Given the description it was obvious that the level of Hb was due to persistent intraabdominal bleeding with some broken artery. What to do?: to carry out an urgent laparotomy, maintaining stabilized the patient with non blood expansors, to explore the liver and to tie the bleeding artery. Of not having added infection, it was convenient reinfuse (autologous blood transfusion), to the patient her own intrabdominally spread blood (500 ml of blood was enough). III) Although the patient was finally convinced to be operated with previous infusion of only red blood cells of a donor, it is already time that for cases like this the main hospitals of our country must have ® Oxycite (liquid perfluorhydrocarbon that carry out efficiently oxygen), in humans.

TRASFUSIONES SANGUINEAS y TESTIGOS de JEHOVA

A través de RPP (Radioprogramas del Perú. 7, 30 pm. 20/2/09), nos enteramos del súbito trauma hepático (rotura y sangrado de hígado), de una mujer de 23 años internada en un hospital de Trujillo-Perú, que con 2 g de hemoglobina (Hb), se negaba a que le transfundiesen sangre total, tendiente a estabilizarla mientras la operaban. Por RPP hablaron familiares cercanos respaldando la posición de la paciente (seguidora de la secta Testigos de Jehová, que prohíbe transfusiones de sangre humana), médicos, abogados y fiscales obstinados en transfundirle sangre total porque la paciente se moría. Todos defendiendo la posición de los médicos, ninguno tratando de entender el aparente absurdo de la paciente. No es la primera véz, que sucede esto en el Perú. Aunque llamamos varias veces a RPP, desistimos porque esta radio no es de libre participación, no es como en los blogs.

I) Importaba conocer el estado de gravedad de la paciente (presión arterial, frecuencia respiratoria, pulsaciones, diuresis, etc). No raramente vemos pacientes con 2 g de Hb (con insuficiencia renal, multiparasitosis, anemia aplástica, etc) caminando, sin riesgo de vida. Aunque estos son pacientes crónicamente adaptados, en ciencia importa demostrar porque alguien tiene su vida en riesgo. II) Siempre importa reflexionar sobre el eje del proceso u enfermedad y en posibles alternativas. Dada la descripcción, era obvio que el nivel de Hb, se debía a sangrado intraabdominal persistente con alguna arteria rota de por medio. Que hacer : realizar una laparotomía de urgencia, manteniendo a la paciente estabilizada con expansores no sanguíneos, explorar el hígado y ligar la arteria sangrante. De no haber infección agregada, era conveniente reinfundir (autotransfusión), a la paciente su propia sangre esparcida intrabdominalmente (bastaban 500 ml). III) Aunque finalmente se convenció a la paciente a ser operada previa transfusión de glóbulos rojos (no sangre total), de un donante, ya es tiempo que para casos como este los principales hospitales del país cuenten con ® Oxycite (perfluorocarbono liquido, que trasporta eficientemente oxigeno), en humanos.

INCOMPLETENESS THEOREM


Unique creations of human beings are arts and math. Great parte of human civilization has been built with the latter. With Principia Mathematica, B. Russell tried to construct a century ahead advanced math, until Wittgenstein observed that Principia statements was nothing but daughters of logic. The true is that many math systems are based on theorems proved by inferential sets of rules and axioms without additional assumptions, in which the derivation of a theorem is interpreted as evidence of truth, without further discussion, although other deductive systems achieve different interpretations. It was not surprising then that in 1931 Kurt Gödel, noted that in any formal math system was impossible to prove all truths contained in it (Incompleteness theorems : Any effectively generated theory capable of expressing elementary arithmetic can not be both consistent and complete. In particular, for any consistent, effectively generated formal theory that proves certain basic arithmetic truths, there is an arithmetical statement that is true, but probably not in the theory). Later, Heisenberg, said that trying to improve the accuracy of the position of an object, inevitably degrades the level of certainty of the momentum.

Now David H. Wolpert (NASA Ames Research Center), argues that understanding physical universe, is beyond any intellect, because it can not be completely understood by any inferential (inductive), system. Wolpert used Cantor diagonalization to prove that any spatial sequence of events (world-lines) consistent with the laws of physics, can not be grasped by any inferential machine to predict all the conclusions of any observation or possible experiment. Equations (laws) that govern complex systems are impossible to infer. We know some laws, not all. Remember that the theory of everything is based on formal math systems. To support his theory, Wolpert established rules to measure quantities, visible phenomena, prediction of future systems or rules to remember past information with laws independent of physics, in which certain questions of the universe must necessarily be specified. Wolpert, uses mathematical entities that he called inferential devices, which operate on a set of possible universes similar to ours, where people are devices or parts of an inferential. According to Wolpert in any system of universes exist quantities that can not be determined by any inferential devices.

TEOREMA de la INCOMPLETUD

Las únicas creaciónes del ser humano son las artes y las matemáticas. Con las últimas, se ha construido la civilización actual. Con Principia Matemática, B. Russell pretendió construir una matemática para las centurias siguientes, hasta que Wittgenstein le encaró que estas eran nada más que hijas de la lógica. Lo real es que muchos sistemas matemáticos asientan sobre teoremas probados por sets de reglas inferenciales y axiomas, sin asunciones adicionales, en las que la derivación de un teorema se interpreta como prueba de verdad, sin mayor discusión, aunque otros sistemas deductivos logren interpretaciones diferentes. No era de extrañar entonces que en 1931 Kurt Gödel, observara que en cualquier sistema formal matemático era imposible probar todas sus verdades contenidas (Incompleteness theorems ; Any effectively generated theory capable of expressing elementary arithmetic cannot be both consistent and complete. Más tarde, Werner Heisenberg, notaria que mejorar la precisión de la posición de un objeto, inevitablemente degrada el nivel de certeza de su momento.

Ahora David H. Wolpert, (NASA Ames Research Center), sostiene que la comprensión del universo fisico, esta más allá de cualquier intelecto. Que este no puede ser completamente comprendido por ningún sistema inferencial (inductivo). Wolpert emplea la diagonalización de Cantor para probar que en cualquier secuencia espacial de eventos (world-lines) consistentes con las leyes de la física, no existe ninguna máquina inferencial capaz de predecir todas las conclusiones para cualquier observación o experimento posible. Las ecuaciones (leyes) que rigen muchos sistemas complejos son imposibles de inferir. Podemos conocer algunas leyes, no todas. Y pensar que la teoria del todo asienta sobre sistemas de matemáticas formales. Para sostener su teoria Wolpert ha creado medidas para medir cantidades, observar fenómenos, predecir sistemas futuros o recordar información pasada con descripcciones independientes de las leyes de la fisica, en las que ciertas cuestiones del universo deben ser necesariamente, espeficadas. Wolpert, emplea entidades matemáticas denominadas por él dispositivos inferenciales, que actúan sobre un set de posibles universos, similares al nuestro, donde las personas son dispositivos inferenciales o partes de uno. Según Wolpert en cualquier sistema de universos existen cantidades que no pueden ser determinadas por cualquier dispositivo inferencial.

TRANSHUMANISM


I) We see daily in hospital beds in Peru adult patients, for which little can be done. Worn bodies (after birth), with various handicaps (chronic malnutrition, stress, inadequate diet, illness, etc.) make them difficult effective treatments or lasting restorations. Contrary to what trumpeted by the Health Ministry, peruvian hospitals are seat mostly of curative practices. A waste -for an underdeveloped country- which should redirect its spending to programs of preventive medicine. Peru, a country rich in food, climate and potential money is bad because is led by people not accustomed to plan wisely and to think that the final purpose of a democratic state is the human being. II) While we counter the benefits of universal health system, fortified food for all, global immunizations, preventive mental health, better work conditions, eradication of preventable diseases, annual medical checkups, preventive sport, etc, we thought if that would suffice.

III) Not for transhumanists who argue that humanity today, need not be the endpoint of evolution. They agree with visions that foster improvement of the human condition. Therefore, they are open to the advances in technology, genetic engineering, molecular nanotechnology, artificial intelligence, etc., designed to dramatically extend our lifespans, to eliminate suffering, improve the physical, emotional, intellectual, economic, social, institutional, cultural, etc. They advocate the elimination of all diseases, including the reversal of aging and removal of accumulated bodily harm, a fine control of human motivations and emotions, uploads of our our minds (memory, personality), in supercomputers, resuscitation of patients cryonically suspended. Facilities to become post-humans with higher body and mental capacities. Small nanobots (robots), implanted in the brain make us smarter and healthier said Ray Kurzweil. Live with greater vigor and mental body, read more books per second, develop a stronger, pure and safer love, says Nick Bostrom. An ethical use of science and technology to expand our capabilities and stay healthy forever.

TRANSHUMANOS

I) Diariamente vemos en camas de hospitales del Perú, enfermos adultos por los que poco se puede hacer. Cuerpos desgastados (poco después de nacer), por diversos hándicaps (desnutrición crónica, estrés, dieta inadecuada, enfermedades laborales, etc), hacen difícil en ellos tratamientos efectivos o rehabilitaciones duraderas. Contrariamente a lo pregonado por el Ministerio de Salud, los hospitales peruanos, son asiento mayoritario de prácticas de medicina curativa. Un derroche para un país subdesarrollado, que debiese redireccionar sus gastos a programas de medicina preventiva. El Perú, país rico en alimentos, climas y dinero potencial, tiene el mal sino de ser dirigido por personas no acostumbradas a planificar sensatamente ni a pensar que el último fin de un estado demócrata es el ser humano. II) Mientras enumerábamos los beneficios de sistemas de salud universales, alimentos fortificados para todos, inmunizaciones, salud mental preventiva, mejoras laborales, erradicación de enfermedades infecciosas prevenibles, chequeos médicos anuales, deporte preventivo, etc, reflexionábamos, si con ello bastaría.

III) No -al menos- para los transhumanistas que arguyen que la humanidad actual, no tiene que ser necesariamente el punto final de la evolución. Ellos son partícipes de cosmovisiones favorecedoras de mejoras de la condición humana. Por ello, son abiertos a los avances de la tecnología, ingeniería genética, nanotecnología molecular, inteligencia artificial, etc, tendientes a extender radicalmente la vida humana, eliminar sufrimientos innecesarios, incrementar las capacidades físicas, emocionales, intelectuales, económicas, sociales, institucionales, culturales, etc. Abogan por la eliminación de todas las enfermedades, por la reversión del envejecimiento y remoción de daños acumulados corporales, por un control fino de las motivaciones y emociones humanas, almacenamiento virtual de nuestras mentes (memoria, personalidad), en supercomputadoras, reanimación de pacientes suspendidos criónicamente. Facilidades para convertirse en post-humanos con capacidades más vastas. Pequeños nanobots (robots), implantados en el cerebro nos harían más inteligentes y sanos dice Ray Kurzweil. Vivir con mayor vigor corporal y psíquico, leer mas libros/segundo, desarrollar un amor mas fuerte, puro y más seguro, agrega Nick Bostrom. En suma un uso ético de la ciencia tecnología para expandir nuestras capacidades y mantenernos sanos por siempre.

CREATING a MINIATURE STAR on EARTH


The british physicist Brian Cox tries to generate nuclear fusion on earth, imitating those generated by nature in different stars whose energy comes from the union of 2 atoms of Hydrogen, that become Helium. Cox thinks that global energy crisis could be solved if we are capable to build a flow of ministars on earth. For it, we have to our disposal inert fusion worked in the National Ignition Facility (NIF) of California, in which deuterium-tritium nanopellets are overheated with a unique laser beam of short life and 500 trillions Watts. The nanopellets collapses and detonate producing a mini-star during a second fraction. Another device: the Z-machine located in New Mexico, channels 1,5 trillion Watts through a spider's web of fine wires that surround nanopellets of De-Tri. In this case, when a mini big crunch happened (Z-pinch), a star is born. If we can create a continuous flow of ministars it will be possible to create a big station of power in the Earth.

These are alternative more cheaper than the JET (Joint European Torus/nuclear fusion reactor of Oxfordshire) or the French ITER that heat De and Tri to 100 million degrees generating nuclear fusion. In the interior of JET and ITER, it is tried to stay the gas so hot so electrons are separated from their atomic nuclei to form plasm sustained in internal torus by intense magnetic fields. If this plasm rich in De and Tri is maintained by long time to high temperatures and pressures it will be obtained abundant Helium (clean energy), giving definitive solution to the energy world crisis.

CREANDO MINIESTRELLAS en la TIERRA

El fisico Brian Cox intenta generar fusión nuclear en la tierra, imitando las generadas por la naturaleza en distintas estrellas, cuya energía proviene de la unión de 2 átomos de H, que se convierten en Helio. Cox piensa que la crisis energética global es solucionable si se logra construir un flujo de miniestrellas en la tierra. Para ello está a nuestra disposición la fusión inerte trabajada en el National Ignition Facility (NIF) de California, en el que nanobolas de deuterio-tritio son sobrecalentadas con un único rayo láser de corta vida de 500-trillones de Watts. Las nanobolas colapsan y detonan produciendo una mini-estrella durante una fracción de segundo, semejando. Otro dispositivo: la máquina-Z ubicada en New Mexico, canaliza 1,5 trillones de Watts a través de una telaraña de finos alambres que rodean a nanobolas de De-Tri. En este caso, al ocurrir un big crunch (Z-pinch), nace una estrella. Si se pudiese crear un flujo continuo de miniestrellas seria posible crear una estación de poder en la Tierra.

Estas son alternativas más económicas que el JET (Joint European Torus/reactor de fusión de Oxfordshire) o el ITER francés, que calientan De y Tri a 100 millones de grados generando fusión nuclear. Al interior del JET y el ITER, se intenta mantener el gas tan caliente de modo que los electrones sean separados de sus núcleos atómicos para formar plasma sostenido en torus internos por intensos campos magnéticos. Si este plasma rico en De y Tri es mantenido por largo tiempo a grandes temperaturas y presiones se obtiene abundante He (energía limpia), dando solución definitiva a la crisis mundial energética.

KURZWEIL : SINGULARITY UNIVERSITY


While in Peru, we battle to assemble public schools that appropriately face the present (that fast becomes past), in other countries schools are built to face the defiant future (right decision). There, the formidable schools of mathematics created by Neil Turok in all Africa (from where will emerge the next Einstein). Now other dreamer: Ray Kurzweil will lead a school (or Singularity University), in which areas related with technologies of exponential growth will be studied, linked and applied to big futurist challenges whose details will be announced today in the annual conference Technology, Entertainment and Design (TED) in Long Beach California. Initially the school will use buildings of Nasa and Ames Research Center, near the offices of Google, Yahoo, Intel Corp and Cisco Systems.

A course of 9 weeks in the Singularity University will cost $25,000 opening its doors to 30 students this summer. During the first 3 weeks 10 subjects will be studied including artificial intelligence, nanotechnology, biotechnology, pandemics, global health, etc. In the following 3 weeks one topic will be studied in detail and in the last 3, a special project will be worked. The University will also offer the whole year courses of 3 and 10 days for business executives offering them a tiny piece of the future of their respective industries. The University is backed by X Prize Foundation, Google and NASA.
In 2005 Kurzweil published his controversial book: The Singularity is Near, in which he exposes the exponential advance of technology that will transform our society in another more intelligent. Computer science's advance will promote rapids advances in other fields, solving together diverse problems: climatic change, poverty, famine and illness. In a previous book Kurzweil, predicted the existence of nanobots that will patrol our blood vessels repairing them, always maintaining our body young. Kurzweil that consumes regularly more than 100 pills of vitamins each day and check regularly 50 indicators of his health, also wrote Fantastic Voyage: Live Long Enough to Live Forever, in which humans live long lives. As it should happen in the next 3 decades, Kurzweil tries to stay healthy until biotechnology enlarge his lifespan and revert his aging.

Mientras en el Perú, batallamos por ensamblar escuelas públicas que enfrenten adecuadamente al presente (que raudamente se convierte en pasado), en otros lares se construyen escuelas para enfrentar al desafiante futuro (decisión correcta). Ahí están las formidables escuelas de matemáticas creadas por Neil Turok en todo Africa (de donde se espera emerja el próximo Einstein). Ahora otro soñador: Ray Kurzweil liderará una escuela (Singularity University), en la que se estudiarán áreas relacionadas con tecnologías de crecimiento exponencial, conexionándolas y aplicándolas a los grandes desafíos futuristas, cuyos detalles serán anunciados hoy en la conferencia anual Technology, Entertainment and Design (TED) en Long Beach California. Inicialmente la escuela usará edificios de la Nasa y el Ames Research Center, cerca de las oficinas de Google, Yahoo, Intel Corp and Cisco Systems.

El 2005 Kurzweil publicó su controversial libro The Singularity is Near, en el que expone el avance exponencial de la tecnología que transformará nuestra sociedad en otra más inteligente. El avance de la informática promoverá rápidos avances en otros campos, resolviendo juntos diversos problemas : cambio climático, pobreza, hambre, y enfermedad. En un libro previo Kurzweil, predijo la existencia de nanobots patrulleros de vasos sanguineos reparándolos, manteniendo nuestro cuerpo siempre joven. Kurzweil, que consume más de 100 pildoras de vitaminas al dia y chequea regularmente 50 indicadores de su salud, también escribió Fantastic Voyage: Live Long Enough to Live Forever, en el que humanos viven largas vidas. Como esto debe ocurrir en las siguientes 3 décadas Kurzweil trata de mantenerse sano hasta que la biotecnologia enlentezca su vida y revierta su envejecimiento.

RELIGIOUS CRITICS to DARWIN


If Copernicus, Descartes and Galilean works unchained hot polemic, the Origin of Species of Darwin, continues dividing the current intellectual and religious world. If in the last decades the theory of the evolution was the most solid base in biology, in the last years, critics religious currents question it with hardness. Let us see: I) the base of the Darwinist argument is the presumed existence of infinitely variable biological features, some inheritables. Having the Earth millions of years of existence, would have existed more species than the acquaintances. Lucy and Tiktaalik, would be hardly some of these gradations. Complex organs would have been emerged for accumulation of countless small variations, being preserved -for instinct- the best.

But, the current fossil registration, doesn't exhibit such variations. Hardly 250,000 fossils of different species of plants and animals stored in diverse museums of the world against 1.5 million current alive species. Darwin argued that it was due to imperfection of geologic record. But, Michael Denton -for whom the fossil record has been totally explored and documented- sustains the existence of 79.1% of fossils belonging to 329 alive families of terrestrial vertebrates. And, when the birds were excluded, the percentage rose at 87.8, concluding that few examples of evolutionary transition existed. Of another side, the structure of discovered fossils is very complex and little gradualist.

According to Darwin, the work of evolution is slow gradual and continuous, where a complete lineage of ancestors mixes imperceptibly with next generation being able that transition be reconstructed in all alive animals. The common thing nevertheless, is the frequent appearance of sudden forms of life, well conformed. Sthepen Gould sustains the existence of evolutions hurried in small and isolated populations that ended up being dominant (punctuated equilibrium). According to him, the most numerous species vary quicker, as long as the intermediate ones -of smaller number- are supplanted and exterminated. Gould proposes 2 non gradualist (gradual evolution of one species in other), characteristics in support of the evolutionary theory: Stasis: disappearance of species with absence of evolutionary direction and morphological changes and Sudden appearance, in full forms. Darwin sustained that species are not mobilized until being considerably modified and improved. In such a point, if they are discovered in a geologic formation, they will appear as if has been created of sudden, being classified as new species.

II) Darwin also argued that we descend from a common ancestor little evolved: a very primitive cell. Nevertheless as it demonstrates the study of Archea (older bacterias), these are extraordinarily complex (complexity, selforganizatión, versatile molecules employees in a myriad of different capacities). Not finding fossil of simple life, generates space for an Universal Designer or, God. The Cambrian Period is special since it represents extensive forms of well preserved life: spongy, worms, brachiopodes, mollusks, primitive arthropods (trilobites), a big bang of natural creativity of the whole animal Kingdom with presence of emerged multicellular animals of sudden. Creationists argues that they were created for heaven's sake, adding that mezozoic era of reptiles until the age of mammals, would represent the interval of time among the 2 parts of biblical Genesis.

For creationists, the fossil discoveries represent: a-forms of complex life, appeared of sudden b- multiplication with limited variety inside the same species c-absence of transitions between different species and d-absence of partial features (new extremities, bones or organs). For our side we believe that reality is one and interpretations, varied. Although we feel close to evolutionist theory, we see the creationist point of view different and influential to the point of inducing many American students to think that evolution is in crisis (Michael J Behe) and to others to think that incompatibility doesn't exist between science and religion.

OPOSICION CRITICO- RELIGIOSA a DARWIN.

Si por cuestionar el status quo científico de su época, las obras de Copérnico, Descartes y Galileo desencadenaron lacerantes polémicas, el Origen de las especies de Darwin, continúa dividiendo al mundo lego e intelectual actual. Si en las últimas décadas la teoría de la evolución era la base más sólida de la biología; en los últimos años, críticas corrientes religiosas la cuestionan con dureza. Veamos: I) Base de la argumentación darwinista, es la presunta existencia de rasgos biológicos infinitamente variables, algunos heredables. Teniendo la Tierra millones de años de existencia, habrían existido muchísimas más especies que las conocidas. Lucy y Tiktaalik, serían apenas algunas de estas gradaciones. Organos complejos habrían sido construídos por acumulación de innumerables pequeñas variaciones, preservándose -por instinto- las mejores.

Empero, el registro fósil actual, no exhibe tales variaciones. Apenas 250,000 fósiles de diferentes especies de plantas y animales depositadas en diversos museos del mundo contra 1.5 millones de especies vivas actuales. Darwin arguyó que ello se debía a la imperfección del registro geológico. Empero, Michael Denton -para quien el registro fósil ha sido completamente explorado y documentado- sostiene la existencia de un 79.1% de fósiles pertenecientes a 329 familias vivas de vertebrados terrestres. Y, cuando se excluyeron las aves, el porcentaje se elevó a 87.8, concluyendo que se tienen pocos ejemplos de transición evolutiva. De otro lado, la estructura de los fosiles descubiertos es muy compleja y poco gradualista.

Según Darwin, el trabajo de la evolución es lento gradual y continuo, donde un linaje completo de ancestros se mezcla imperceptiblemente con la siguiente generación pudiendo tal transición ser reconstruida en todos los animales vivos. Lo común no obstante, es la frecuente aparición de súbitas formas de vida, bien conformada. Sthepen Gould sostiene la existencia de evoluciones aceleradas en pequeñas y aisladas poblaciones que luego llegaron a ser dominantes (equilibrio punteado). Según él, las especies más numerosas varían más rápido, en tanto las intermedias -de menor número- son suplantadas y exterminadas. Gould propone 2 características no gradualistas (evolución gradual de una especie en otra), en apoyo de la teoría evolutiva :Estasis :desaparición de especies con ausencia de cambios direccionales evolutivos y morfológicos y Aparición súbita, de formas completas. Gould sostiene que las especies no son movilizadas hasta ser considerablemente modificadas y mejoradas. En tal punto, si son descubiertas en una formación geológica, aparecerán como si hubiesen sido creadas de súbito, siendo clasificadas como nuevas especies.

II) Darwin arguía además que descendemos de un ancestro común poco evolucionado : una célula muy primitiva. No obstante como lo demuestra el estudio de las arqueas (bacterias más antiguas), éstas son extraordinariamente complejas (complejidad, autorganización, moléculas versátiles empleadas en una miríada de diferentes capacidades). El no encontrar fósiles de vida simple, genera espacio para un Diseñador Universal o, Dios. El Periodo Cambriano es especial ya que representa extensas formas de vida bien preservada : esponjas, gusanos, braqueópodos, moluscos, artrópodos primitivos (trilobites), un big bang de creatividad natural de todo el reino animal con presencia de animales multicelulares emergidos de súbito. Los creacionistas arguyen que fueron creados por Dios, agregando que la era mezozoica de los reptiles hasta la edad de los mamíferos, representaría el intervalo de tiempo justo entre las 2 partes del Génesis bíblico.

Para los creacionistas, los hallazgos fósiles representan: a-formas de vida compleja, aparecidas de súbito b- multiplicación con variedad limitada dentro de su misma especie c-ausencia de formas transicionales entre diferentes especies y d-ausencia de rasgos parciales (nuevas extremidades, huesos u órganos). Por nuestro lado creemos que la realidad es una y las interpretaciones varias. Aunque nos sentimos cercanos a la teoría evolucionista, admitimos la interpretación creacionista como diferente e influyente al punto de inducir a muchos estudiantes americanos a pensar que la evolución está en crisis (Michael J Behe) y a otros a que no existe incompatibilidad entre ciencia y religión.

PERUVIAN MINE COLLAPSING


According to Mario Huamán (CGTP) and the Peruvian priest Marco Arana, the mining company Casapalca (Main Central Roadway. Km :115/Huarochirí-Lima), dedicated to copper, silver, lead and zinc extraction, have most of their workers to services modality, lacking of social security and right to pensions. They make them contract every 3 months and it is not permitted to be entitled to unions. Also, the company would not fulfill the devices that avoid labor risks. The conditions in which they live are very bad. The accusations are serious, because 7 days ago happened a collapse that buried under earth (100 m), to 5 miners that extracted zinc, for causes that are under investigation. According to Alberto Alta, manager of Administration and Finances of Casapalca, the collapse was due to the natural explosion of geologic rocks in a vertical descent (piques, chimney). Contrarily the representative of the workers of the mine Antonio Quispe (47), said that the collapse was due to a not well planned detonation, in an place with absence of geological studies and measures of security. According to him, the detonation destroyed the structures used as stairway to arrive to the surface. While are carried out the studies of the accident, let us meditate:

1-Although the collapses are not uncommon in all world’s mines, the strict adoption of measures of security, diminish human mortality. After a collapse earth removal with heavy machinery and employment of dogs scanners should find the survivors in 48 hs. For it, it is necessary to work day and night, without rest. In this case, were found 3 death corpses, 5 days later. With more days, the survival is difficult, unless the miners had oxygen tubes, well preserved foods, water frequently renovated, communication device with the surface, elements of first aids, personal illumination and nets of compressed air. 2-¿Was built the pique according to mining international security: a cord of security, another for ascent and descent of the personnel, another to ascend and descend materials, access stairway, work mesh scaffold of security?. Did the pique have protection systems to avoid the fall of people, objects or materials, toward lower levels?. Was studied the composition of the geologic strata of the pique?¿Had the pique structural flaws or aquifer caves?. Was the pique built on the roof of a gallery -instead of lateral areas?- Did the piques have appropriate inclination to impede the fall of rocks toward tunnels or access levels?.

DERRUMBE de MINA en PERU

Según Mario Huamán (CGTP) y el sacerdote peruano Marco Arana, la empresa minera Casapalca (km115/Carretera Central/ Huarochirí-Lima), dedicada a la extracción de cobre, plata, plomo y zinc, tiene a la mayoría de sus trabajadores bajo la modalidad de services, carentes de seguro y derecho a pensiones. Les hacen contrato cada 3 meses y no tienen derecho a sindicatos. Además, no cumpliría con los dispositivos que evitan riesgos laborales. Las condiciones en que viven serian paupérrimas. Las denuncias adquieren relieve, porque hace 7 días ocurrió un derrumbe que sepultó bajo tierra (100 m), a 5 mineros que extraían zinc, por causas que se investigan. Según Alberto Alta, gerente de Administración y Finanzas de Casapalca, el derrumbe se debió al estallido natural de rocas de placas geológicas de un descenso vertical (pique, chimenea). Contrariamente el representante de los trabajadores de la mina Antonio Quispe (47), dijo que el derrumbe se debió a una detonación mal planificada, en un pique con ausencia de estudios sobre resistencia de estratos y medidas de seguridad. Según él, la detonación destruyó las estructuras que usaban como escalera para llegar a la superficie. Mientras se realizan los estudios del accidente, reflexionemos:

1-Aunque los derrumbes no son infrecuentes en todas las minas del mundo, la adopción estricta de medidas de seguridad, atenúa la mortalidad humana. Trás un derrumbe la remoción de tierra con maquinaria pesada y el empleo de canes rastreadores debe encontrar a los sobrevivientes en 48 hs. Para ello, hay que trabajar dia y noche, sin descanso. En este caso, se encontraron 3 cadáveres descompuestos, 5 dias después. Mas allá, la supervivencia es difícil, a menos que se cuente con tubos de oxigeno, alimentos no perecibles, agua potable frecuentemente renovada, equipos de comunicación con la superficie, elementos de primeros auxilios, iluminación personal y redes de aire comprimido. 2-¿Fué construido el pique, con normas de seguridad internacional minera : un cordel de seguridad, otro para ascenso y descenso del personal, otro para subir y bajar materiales, escalera de acceso, andamio de trabajo, malla de seguridad?. ¿Contaba con canastillas antiderrumbes, sistemas de protección para evitar la caída de personas, objetos o materiales, hacia niveles inferiores?. ¿Se estudió la composición de los estratos geológicos del pique?¿tenia fallas estructurales o cuevas acuíferas?. ¿Fué construido sobre el techo de una galería -en vez de, sobre áreas laterales?.¿Contaba con inclinación adecuada para impedir la caída de rocas sobre socavones o niveles de acceso?.

CAPECCHI en LIMA: LO BUENO, LO MALO, LO FEO.


-10/2/09: Estuvieron presentes en el Centro de Convenciones (CC), del Colegio Médico del Perú (CMP), importantes médicos peruanos. Todos querían escuchar a Mario R. Capecchi. Al ingresar al CC, Capecchi fué seguido por una veintena de médicos, que ansiaban tomarse fotos con él o simplemente darle la mano, para sentirse al menos por unos segundos -algo de Premio Nóbel-. A nivel de falencias, era notoria la ausencia de otra pizarra adyacente a la principal para traducción escrita instantánea al español, para aquellos que no contaban con auriculares inhalámbricos. También faltó retrasmitir la charla de Capecchi, a nivel nacional usando multimedia.

-El primer speech corrió a cargo del Jefe de Hematología del Hospital E. Rebagliati M, quien refirió técnicas de trasplante de médula ósea en pacientes con enfermedades hematológicas. Al hablar Capecchi poco después, desnudó nuestras falencias. Si bien en los trasplantes de medula ósea, también se trasplantan células madre, éstas técnicas están deviniendo en obsoletas (no son efectivas por encima de los 40 años, ocasionan enfermedad del injerto contra el huésped, etc). Hoy se habla de trasplante de células madre embrionarias, trasplante de células madre inducidas a partir de células adultas empleando 4 genes, de células madre mesenquimales capaces de reforzar nuestro sistema inmune. De tipos de células madre, de la función de genes en las células madre, del reemplazo de genes defectuosos por otros artificiales intraútero. Capechi está en el piso 300, nosotros en el primero.

-También cierto desconocimiento del tema. No estamos seguros que todos los asistentes comprendiesen a cabalidad lo divulgado por Capecchi. El tríptico del CMP decía : Seminario Internacional : Investigación de células madre. Avances, controversias, perspectivas. Capecchi no es especialista en células madre sino en química, física, biofísica y genética. Su trabajo es crear genes (capaces de insertarse en genomas de células madre embrionarias mediante recombinación homóloga), inhibitorios de funciones normales en organismos en desarrollo (ratones). El titulo debió ser : Perspectivas de la recombinación homóloga genética artificial en ratones, a objeto de conocer la función de órganos en desarrollo.

-El Presidente del CMP, tuvo frases laudatorias para Capecchi, dedicando gran parte de su speech a destacar las cualidades morales y de trabajo del italo-americano. Aunque no estamos en contra de este tipo de discursos de bienvenida, tampoco estamos seguros de si las circunstancias merecían tal disgresión. ¿No era más importante trazar un esbozo de una red nacional de acopio de sangre de cordón umbilical (que se elimina sin mas ni más), solicitando el apoyo del Premio Nóbel ?.

De Capecchi un calco de la personalidad práctica de Arthur Kornberg, padre, no se espere actitudes filosóficas a profundidad. No es un Einstein ni un Schrödinger. Capecchi dice que a él le gusta más hacer, que hablar. Antes de iniciar su charla, manifestó su asombro por las ruinas de Machu Picchu y el Cusco, que visitó el día anterior. Hecho importante. Más adelante lo invitamos al Cusco por 1 més, ejecutando simultáneamente un curso teórico-practico de talleres de genética recombinante. ¿Hubo alguien que coordinara viajes de biólogos y médicos jóvenes a trabajar con Capecchi?. Se podria haber estructurado un mini TLC científico con el, ligándolo a nosotros. Más que admirarlo era importante trabajar con él.

-Dia 11/2/09. En su tercer speech, Capechi dijo que en los próximos años, continuará perfeccionando lo avanzado. Trabaja con ratones porque estos comparten un 99% de sus genes con los humanos. Sus genes modificados tienen ahora o color rojo o, color verde, siendo más fácil distinguirlos. Con ellos ha logrado diferenciar 2 tipos de células madre (las migrantes y las dormidas). Casi tiene ya un mapa funcional completo del sistema nervioso de ratones. No obstante necesita descubrir nuevos y más específicos biomarcadores. Capecchi está seguro que sus técnicas curaran la diabetes, el Alzheimer, la hemofilia y otros.

-Por la tarde -casi al final- Capecchi dijo que no hay edad para aprender. Lo importante es que nuestro cerebro siempre esté en actividad. No es malo empezar de cero, el nuevo no tiene prejuicios y sus visiones nadie las imagina. Hoy la gente trabaja mucho, en lugar de hacer algo que los emocione.

PERUVIAN DEATHS CAUSED by PSEUDOMONA AERUGINOSA


Deaths caused by resistant bacterias to antibiotics, acquired in several hospitals go this way:
-2001. hospital Regional hospital of Ica: 2 babies die for Pseudomona aeruginosa.
-2004. Hospital of Support of Cusco. 11 babies die for Klebsiella. Inner hospital infection attacked to 25% of the babies.
-2005. Hospital of Support of Cusco. It was discovered the presence of 3 bacterias in diverse hospital areas: Enterobacter, Serratia and P. aeruginosa.
-2007. Hospital Hipólito Unanue. 7 newly born death, possibly for P. aeruginosa.
-2009. 2 women die: a) Doris Mallqui G -36 years –she entered the 6/1/09 for a severe intestinal infectious. Hospitalized she gets complicated with a severe urinary infection, another in her breast protesis, another in her lung (P.aeruginosa), dying the 1/2/09 b) María E. Venegas -72 years-. She entered to the hospital, the 1/8/08 for appendicitis, remaining hospitalized by 4 1/2 months. When she began to develop ulcers full with pus, she entered in coma. While cultivations of pus were diagnosed of P. aeruginosa in a private clinic, the H. Casimiro Ulloa of Lima, diagnosed Klebsiella. For these days, died in Brazil, the model Mariana Bridi da Costa, diagnosed of renal calculations, complicated with urinary infection and later on with sepsis for P. aeruginosa, resistant to diverse antibiotics. She was amputated her hands and feet, dying soon after. How avoiding these deaths and the infection?.

I) As long as P. aeruginosa (bacteria gram-negative), is ubiquitous we cannot trust to eliminate it using single antibiotics. The bacteria, consumes great variety of organic compounds, colonizing inhospitable niches. The bug is present in low quantities in our foods. 2-8% of our feces contains it. The germ secret toxic compounds that cause extensive tissue damage and interfere with the operation of the immune system. According to Ciro Maguiña, alone 30% of stumps of P. aeruginosa is sensitive to the most potent antibiotics, fact that maybe due to the presence of great number of transporters that expel the drugs out of the cell. The correct thing is not to eliminate the bacterias with bactericide antibiotics, but cohabiting with them, maintaining an appropriate nutrition, reinforcing our immune system, following strict rules of hygiene.

II) The other thing has to do with specialized periodic visits to ICU (intensive care unit), Emergency, Unit of Burnt, rooms of patients with terminal cancer, rooms of babies, advanced age, cultivating germs of probes, catheters, mechanical ventilators. We must follow rules and recommendations established by Infectology. The high letality that causes, forces to consider the bug as a problem of public health, demanding that antibiotics be prescribed by specialist doctors, according to the cultures that has the patient or to the surveillance that is carried out in the hospitals. III) The common sense tells us that rooms or ICU, where these germs grow should be modified structurally. These, are closed tombs, with restricted ventilation, where all that swarms in their interior (patients, doctors, residents, nurses, technicians, students, machines, etc), can be contaminated. If these patients can be prescribed in their houses, sure they would not adcquire P. aeruginosa. a) To maintain the asepsis (laundry of hands, use of masks and gloves, restrictions to the entrance of strange people), doesn't have much sense in spaces so closed, because in these rooms, babies and adults live in areas of high populational density. b) Old hunter-gatherers didn't get sick with these germs because they occupied spaces of scarce populational density.
PERU: MUERTES CAUSADAS por PSEUDOMONA AERUGINOSA.
Asi van las muertes ocasionadas por bacterias resistentes a antibióticos, adquiridas intrahospitalariamente:
-2001. Hospital Regional de Ica : Mueren 2 bebés por Pseudomona aeruginosa.
-2004. Hospital de Apoyo del Cusco. Mueren 11 bebés por Klebsiella. La infección intrahospitalaria atacó al 25% de bebes.
-2005. Hospital de Apoyo del Cusco. Se determina la presencia de 3 bacterias en diversos ambientes hospitalarios : Enterobacter, Serratia y P. aeruginosa.
2007.Hospital Hipólito Unanue. Muerte de 7 recién nacidos, posiblemente por P. aeruginosa.
2009. Mueren 2 mujeres : a) Doris Mallqui G -36 años- ingresó el 6/1/09 por un severo cuadro infeccioso intestinal. Hospitalizada se complica con una infección urinaria severa, otra en su protesis mamaria, otra a nivel pulmonar por P. aeruginosa, falleciendo el 1/2/2009 b) Maria E. Venegas -72 años-. Ingresa el 1/8/08 por apendicitis, permaneciendo hospitalizada por 4 1/2 meses. Cuando empezaron a salirle heridas en manos, llenas de pus, entró en coma. Mientras cultivos de pus eran diagnosticados de P. aeruginosa en una clínica privada, el H. Casimiro Ulloa de Lima, diagnosticaba Klebsiella. Por estos días, muere en Brasil, la modelo Mariana Bridi da Costa, diagnosticada de cálculos renales, complicada con infección urinaria y posteriormente con sepsis por P. aeruginosa, resistente a diversos antibióticos. Le amputaron manos y pies, muriendo poco después. ¿Como evitar estas muertes y por ende la infección?.

1) En tanto P. aeruginosa (bacteria gram-negativa), es ubicua no podemos confiar en eliminarla empleando solo antibióticos. La bacteria, consume gran variedad de compuestos orgánicos, colonizando nichos inhóspitos. Se halla en bajas cantidades en nuestros alimentos. 2-8% de nuestras heces la contienen. Secreta compuestos tóxicos que causan daño tisular extenso e interfieren con el funcionamiento del sistema inmune. Según Ciro Maguiña, solo el 30% de cepas de P. aeruginosa son sensibles a los antibióticos más potentes, lo que quizás se deba a la presencia de gran número de transportadores que secretan las drogas al exterior de la célula. Lo correcto no es eliminar las bacterias con antibióticos bactericidas, sino convivir con ellas, manteniendo una adecuada nutrición, reforzando nuestro sistema inmune, siguiendo reglas de higiene estrictas.

2) Lo otro tiene que ver con visitas periódicas especializadas a UCI, Emergencia, Unidad de Quemados, salas de pacientes con cáncer terminal, salas de bebes, edad avanzada, cultivando gérmenes de sondas, catéteres, ventiladores mecánicos. Hacer cumplir las Recomendaciones de los Manuales de Infectología. La alta letalidad que ocasiona obliga a considerarla como un problema de salud pública, exigiendo que los antibióticos sean prescritos por médicos especialistas, de acuerdo a los cultivos que tenga el paciente o a la vigilancia que se realice en los hospitales. 3) El sentido común nos dice que las salas o UCI, donde crecen estos gérmenes deben ser modificadas estructuralmente. Estas, son tumbas cerradas, con ventilación restringida, donde todo lo que pulula en su interior (pacientes, médicos, residentes, enfermeras, técnicos, estudiantes, etc), pueden contaminarse. Si estos pacientes pudiesen ser medicados en sus casas no adquiririán P. aeruginosa. a) Mantener la asepsia (lavado de manos, uso de máscaras y guantes, restricciones al ingreso de personas extrañas), no tiene mucho sentido en espacios tan cerrados, porque en estas salas, bebes y adultos moran en areas de alta densidad poblacional. b Los antiguos cazadores-recolectores no se enfermaban con estos gérmenes porque ocupaban espacios de escasa densidad poblacional.

MARIO CAPECCHI in PERU.



The ítalo-american Mario R. Capecchi (Prize Nóbel of Medicine and Physiology/2007), is in Peru for 2 days: 10 and 11/2009. The motto of the Colegio Médico del Perú adduces that he is discoverer of embryonic stem cells and that he had a very hard childhood soon after the II world war. 1) The truth is that although Capecchi works with embryonic stem cells, obtained from blastocysts of mice, his strength does not reside in these cells. He simply uses them as vehicles of genes. The master of that area is Sir Martin J. Evans (Cardiff University/Wales/Medicine, Medicine P.Nobel/2007) who not alone identified them in mice, but rather he finished separating and cultivating them. Capecchi, who looks well to his 70 years, works as researcher in the Howard Hughes Medical Institute/USA. The Prize Nóbel that has a PhD in Biophysics (1967), has worked in Harvard, Cambridge, Utah/Salt Lake City/USA, etc. Capecchi, who is strong in engineering, biophysics and computer science, is convinced that the human body is a biomachine whose informative software is carried out by combinations of genes.

2) Capecchi, knew that bacterial faulty genes were self reparated by means of homologous recombinations in segments of its DNAs. He devised then a way to know the normal function of an in development organism, the normal and pathological body-function processes of mammals, etc ; building in bacterias, specific inactivating genes of normal functions (stop an eye developing, stop formation of a specific protein. Knock out, technique). Then he inserts genes (gene targeting), among specific homologous segments of bacterial DNA. Modified genes are injected (transfection), in millions of cultivated embryonic stem cells of mice obtaining pure cellular stumps with the modified gene. Then he injects modified cells in mice’ s blastocysts, obtaining offsprings composed by transgénic and normal mice. This way, we can identify functional defects, normal functions and properties of specific genes in health and in illness 3) It is clear that using human embryonic cells in few years the function of all the genes will be understood. In that moment thousands of illnesses will be corrected using specific intrauterine genes. The restoration of lost function of certain genes (knock in, technique), was begun by Oliver Smithies (82, british, winner of Medicine P.Nobel/2007), in genetically modified mice’s embryonic stem cells that expressed tallasemias, cystic fibrosis, atherosclerosis,etc.

La propaganda del Colegio Médico aduce que es descubridor de las células madre y que tuvo una infancia muy dura poco después de la II guerra mundial. 1) La verdad es que si bien Capecchi trabaja con células madre embrionarias, obtenidas de blastocistos de ratones. su fuerte no son estas células. El simplemente las usa como vehículos de genes. El maestro de esa área es Sir Martin J. Evans (Cardiff University/Wales/coparticipe del P.Nobel/2007), quien no solo las identificó en ratones, sino que terminó separándolas y cultivándolas. Capecchi a quien se le vé bien a sus 70 años, trabaja como investigador en el Howard Hughes Medical Institute/USA. El Premio Nóbel, que tiene un PhD en Biofisica (1967), ha trabajado en Harvard, Cambridge, Utah/Salt Lake City/USA, etc. Capecchi, fuerte en ingeniería, biofísica e informática, es un convencido de que el cuerpo humano es una biomáquina cuyo software informativo es vehiculizado por combinaciones de genes.

2) Capecchi, sabia que los genes defectuosos bacterianos se autoreparaban mediante recombinaciones homologas de segmentos de sus DNAs. Ideó entonces conocer la función normal de un organismo en desarrollo, los procesos funcionales corporales normales y patológicos de mamíferos, etc, construyendo en bacterias, genes inactivadores de funciones normales especificas (desarrollo de un ojo, formación de una proteína. Técnica : knock out), que luego inserta (gene targeting), entre segmentos homólogos específicos de DNA bacteriano. Despues inyecta los s genes modificados (transfeccion), a millones de células embrionarias cultivadas de ratones obteniendo cepas celulares puras con el gen modificado. Mas tarde inyecta las células modificadas en blastocistos de ratones, obteniendo progenies compuestas por ratones transgénicos y normales. Asi, identificando defectos funcionales se conocen las funciones normales y las propiedades de genes especificos en la salud y en la enfermedad 3) Es claro que empleando células embrionarias humanas en pocos años se comprenderá la función de todos los genes. En ese momento miles de enfermedades se corregirán empleando genes específicos, intraútero. La restauración de la función perdida de ciertos genes (técnica knock in), fué iniciada por Oliver Smithies (82, inglés/coparticipe del P.Nobel/2007), en células madre embrionarias de ratones genéticamente modificados, expresantes de: talasemias, fibrosis quistica, aterosclerosis.

DINOS in PERUVIAN ANDEAN

What important will be for peruvian science the recent discovery of footprints and fossilized remains from dinosaurs at 4600 msnm in Yanacancha/Ancash/Peru?. Several: 1) It is the precise moment for the Concytec (Peruvian scientific Office), to coordinate with the owners of the mine Antamina (discoverers of the remains while they built a road from Canacancha toward a mining camp), in order that the mine give 2 million dollars (1 millón/each year), to exhume, to study and to preserve as much as dinosaur is possible. The Peruvian state would stop to charge to the mine 2 million dollars for taxes. The study must be lead by an Argentinean or American paleontologist (with several international publications), a geologist, 4 Peruvian university professors and students, all appropriately paid. 2) The mine Antamina would coordinate the construction of a Museum of Paleontology containing the footprints and the reconstructed remains in areas near the mine. Some skeletons could be exchanged or sold to foreign museums, as long as few of these, possess full skeletons. The museum together with the lagoon of Conococha and the origin of the river Santa would generate an extra flow of tourists to this area.

3) The found skeletons belong to the Cretácic (145-65 million years b.P). According to some observers the fossils look like sauropterigya remains (semiaquatic lizards of long members), icthyosaur (big marine reptiles with intermediate forms between fish and dolphin (evolved convergently to fishes from terrestrial reptiles), extinct crocodiles, pterosaur (flying reptiles, first vertebrates in conquering the air), turtles, remains of fish and spineless well preserved. Finding fish to 4600 msnm is accountable keeping in mind that during the Cretácic emerged the Mountain of the Andes (from seabed), for subduction of Nazca’s plate under the South American Plate. Also, for the high levels reached by marine waters by these times.

DINOS en ANDES PERUANOS

Que de importante para nuestra ciencia nos podria ofrecer el reciente hallazgo de pisadas de dinosaurios y restos fosilizados de los mismos a 4600 msnm en Yanacancha/Ancash/Peru?. Varios: 1) Es el momento preciso para que el Concytec (Oficina científica peruana), coordine con los dueños de la mina Antamina (descubridores de los restos mientras construían una vía desde Canacancha hacia un campamento minero), a fin de que facilite 2 millones de dólares (1 millón/año), para desenterrar, estudiar y preservar cuanto dinosaurio sea posible. El estado peruano dejaría de cobrarle a la mina 2 millones de dólares por concepto de impuestos. Jefaturarían los estudios, un paleontólogo argentino o un americano, un geólogo, 4 profesores universitarios peruanos y alumnos, todos adecuadamente rentados. 2) La mina Antamina coordinaría la construcción de un Museo de Paleontologia conteniendo las pisadas y los esqueletos recontruídos, en áreas cercanas a la mina. Algunos esqueletos podrían ser intercambiados o vendidos a museos extranjeros, en tanto pocos de estos, poseen esqueletos completos. El museo, de conjunto con la laguna de Conococha y el origen del rio Santa generarían un flujo turístico extra a la zona.

3) Los esqueletos encontrados pertenecen al Cretácico (145-65 millones de años a.P). Según algunos observadores ciertos fósiles semejan restos de sauropterigios (lagartijas semiacuáticas de largos miembros), ictiosaurios (grandes reptiles marinos con formas intermedias entre pez y delfin (evolucionados convergentemente a peces partir de reptiles terrestres), cocodrilos extintos, pterosaurios (reptiles voladores, primeros
vertebrados en conquistar el aire), tortugas, restos de peces e invertebrados bien conservados. El encontrar peces a 4600 msnm es explicable teniendo en cuenta que durante el Cretácico emergió la cordillera de los Andes (desde los fondos marinos), por subducción de la placa de Nazca, debajo de la Placa Sudamericana. También, por los altos niveles alcanzados por las aguas marinas por estas épocas.

CRITICAL THINKING


When I am teaching undergraduate medical students (17-23 years old), I understand that I receive human material used to memorize, but full of potential. My job is to train their minds, giving them critical thinking. I teach Clinical Hematology. Consider my plan: I) After collecting data of case history, physical examination, ancillary tests and others, the students present the patient data. I urged them to explain all data in 3 minutes, which creates commotion in some because they are used to present data in boring exhibitions 1 hour on average. This way, I explore their ability to make abstract of each patient, while I emphasized that all patients carrying 1 or 2 central problems (usually 1), which can be expressed in few and precise words. II) Then I analyze the questions. When students ask, "are his feces black?" (instead of "what color are your stools?”), I emphasized that questions must be neutral and unbiased. Then we analyze objective data. If they say that the left knee is swollen, I demand evidence. If they insist that naked eye is enough, I insist that there is necessary numerical data obtained after passing tapes around the normal and the injured knees, respectively.

III) Then I urge them to develop a list of all the problems found, searching definitions of medical terms, with the help of medical dictionaries and Internet. Then comes the dialectic. I note that in the list of problems there are 1 or 2 major problems and many other minor. The task is to identify the main issues. If we have 3 problems: a) Fever b) abdominal pain and c) palpable lymph nodes. What is the main symptom?. I explain that this is one whose pathophysiology explains the other problems. Or, the minor problems are dependent on the main. In this case lymphoproliferative syndrome, is the main problem. IV) Finally we focus on the problem by analyzing all related data, focusing on functionality and disturbances caused by, looking for explanations based on objective data. Then sharpers swords appear here (teacher’s questions), all dialectical. Most students do not respond initially. Some of those who respond do so by giving false evidence. Other argue with sci-fi explanations. Then we used the Socratic method of questions-answers, using reasonable assumptions and critical thinking to evaluate the structure and consistency of reasoning, with observation, experience, reasoning and the scientific method. What counts here is the clarity, accuracy, precision, evidence. Logic is used to distinguish reasonable from unreasonable, the true from the false. Critical thinking is linked to skepticism and the study and detection of fallacies, needing for this task: open mind, healthy skepticism, intellectual humility, freedom of thought and high motivation. Good critical thinkers are better equipped to make decisions and solve problems. The best: critical thinking, is acquired through training.

PENSAMIENTO CRITICO

Cuando formo alumnos de pregrado de medicina (17-23 años de edad), entiendo que recibo material humano (algo crudo), acostumbrado a memorizar, aunque pleno de potencialidades. Mi labor es formar sus mentes, dotándolas de pensamiento critico. Enseño clínica hematológica. Examinemos mi plan: 1) Trás realizar la anamnesis, el examen físico y anotar los exámenes auxiliares, los alumnos presentan los datos del paciente. Los insto a que expongan todos los datos en 3 minutos, lo que genera conmoción en algunos, porque están acostumbrados a presentar datos en aburridas exposiciones de 1 hora en promedio. Asi, exploro la capacidad de resumen de c/uno, enfatizando que en tanto todo paciente porta 1 o 2 problemas centrales (generalmente 1), sus problemas pueden ser dichos en pocas y precisas palabras. 2) Luego analizo la anamnesis. Cuando los alumnos preguntan : “son sus heces de color negro” (en véz de “de que color son sus heces”), enfatizo que las preguntas deben ser neutrales y sin sesgos. Luego analizamos los datos objetivos. Si dicen que la rodilla izquierda está hinchada, exijo pruebas. Cuando insisten en que se vé a simple vista, insisto que obtengan datos numéricos trás pasar cintas métricas por las rodillas afecta y sana, respectivamente.

3) Luego los insto a desarrollar una lista de todos los problemas encontrados, precisando definiciones de términos médicos, con la ayuda de diccionarios médicos e Internet. Entonces viene la dialéctica. Señalo que en la lista de problemas existen 1 o 2 problemas principales y muchos otros secundarios. La tarea es identificar los problemas principales. Asi, de 3 problemas : a) fiebre b) dolor abdominal y ganglios palpables ¿cual es el síntoma principal?. Les explico que este, es aquel cuya fisiopatología explica los otros problemas. O, los problemas secundarios son dependientes de el. En este caso el síndrome linfoproliferativo, es el problema principal. 4) Finalmente nos concentramos en el problema principal analizando todo los datos relacionados con el, centrándonos en la funcionalidad y alteraciones producidas por el, buscando explicaciones sustentadas en datos objetivos. Cual aguzadas espadas, aparecen aquí, las preguntas del profesor, dialécticas todas. La mayoría de alumnos no responde, inicialmente. Algunos de los que responden lo hacen dando evidencias falsas. Otros suponen cosas. Ingresamos entonces al método socrático de preguntas y respuestas, al área de hipótesis razonadas al pensamiento crítico evaluando la estructura y consistencia de los razonamientos, vapuleando algunas afirmaciones aceptadas como verdaderas, recurriendo a la observación, la experiencia, el razonamiento y el método científico. Lo que cuenta aca es la claridad, la exactitud, la precisión, la evidencia. Se emplea la lógica para distinguir lo razonable de lo no razonable, lo verdadero de lo falso. El pensamiento crítico esta ligado al escepticismo y al estudio y detección de las falacias, necesitandose para esta tarea : mente abierta, escepticismo sano, humildad intelectual, libertad de pensamiento y alta motivación. Los buenos pensadores críticos estan mejor equipados para tomar decisiones y resolver problemas. El pensamiento critico, se adquiere mediante entrenamiento.
